To register for any of the clinics above please follow these simple instructions:
- Click on the registration form button above
for the camp location you would like to attend.
A PDF file will download to your computer for you to print at your convenience.
- Complete the upper part and sign the parent/guardian release at the bottom.
- Mail the registration form with your payment method to the address on the registration form.
